Job Description
Nurses (Case Management) for Whiteriver, AZ and Winterhaven, CA. Position will provide professional nursing case management; purchased referred care and mental health coordinating between inpatient, outpatient care teams, external entities and providing case management activities, coordinating care and supporting mental health, medical, and ancillary services. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
