Job Description
The Registered Nurse (RN) Patient Care Coordinator (PCC) is responsible for overseeing and coordinating patient care activities to ensure safe, effective, and high-quality care delivery. This role serves as a clinical resource, liaison, & patient advocate, facilitating communication among patients, families, interdisciplinary team members, and healthcare providers. The RN PCC supports care planning, monitors patient progress, and helps ensure continuity of care across services and settings.
