Job Description
The Case Management RN coordinates complex Veteran care across inpatient, outpatient, and community settings. Assesses needs, develops care plans, supports self-management, and facilitates safe transitions. Leads interdisciplinary collaboration, patient advocacy, staff education, and evidence-based improvements. Evaluates program outcomes, promotes efficient resource use, and improves quality, safety, access, and patient experience beyond the immediate practice setting and across services.
