Job Description
The Case Management Urology Registered Nurse (RN) delivers competent, evidence-based care to assigned patients. Responsibilities include patient management, care coordination, and discharge planning for both inpatient and outpatient settings. The RN assists Veterans, families, and caregivers in accessing the most suitable options and services for their complex healthcare needs, addressing conditions ranging from acute and chronic to multiple, complex, catastrophic, or life-threatening illnesses.
