Job Description
The VA New Jersey Health Care System is currently seeking an Outpatient Registered Nurse to work Intermittently as a member of our Patient Aligned Care Team (PACT) at the Lyons, New Jersey VA campus. The PACT Outpatient Registered Nurse assesses, plans, implements, and evaluates care based on age specific components, coordinates care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
