Job Description
This position is located at the Kayenta Service Unit in Kayenta, Arizona. This position serves as an inpatient nursing case manager responsible for providing comprehensive case management, utilization management, discharge planning, and care coordination for patients with acute and chronic health conditions and serves as a clinical resource for inpatient services. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
