Job Description
Provides case management services to individuals and communities, ensuring coordinated care, compliance with public health and clinical guidelines, evaluation of outcomes. Develops care plans and serves as liaison to patients, providers, care teams. This position reports to the Director of Communicable Disease Prevention and Control at the Chinle Service Unit, Division of Nursing Services. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
