Job Description
The PACT Care Manager Nurse uses the nursing process and evidence-based practice to collaborate with the team (Patient, Primary Care Provider, PACT RN Care Manager, Health Technician, Pharmacists, and Clerical Associate) and expanded team (family member or caregiver, consulting providers, internal and community-based services involved in the care of the patient) in developing the patient-driven care plan for prevention and management of diseases.
