Nurse-RN-CM (RN Care Manager) -Clinical Resource Hub (CRH)

Accountable for all elements of the nursing process in providing or supervising direct patient care: assessing, planning, implementing, and evaluating age-specific care. Coordinates care across the continuum with a focus on patient education, self-management, and customer satisfaction; follows established policies and guidelines; collaborates with the interdisciplinary team to influence outcomes; participates in unit- or program-level quality improvement and customer service programs.
