The Transition and Care Management (TCM) Nurse Case Managers provide a broad range of case management services to combat Veterans transitioning out of active duty and to other post 9/11 Veterans leaving military service with disabilities. The Registered Nurse will assess, plan, implement and evaluate care based on age-specific components.
