Job Description
This RN admission/discharge coordinator reports to Social Work Service and acts as a primary support for inpatient discharge planning. The role supports transition of care to prevent breakdown at service boundaries. The role's main function is to safeguard high-risk discharges by ensuring comprehensive discharge plans. The role works directly with inpatient interdisciplinary teams, including social work case managers, to review high-risk patients and coordinate safe dispositions.
