Job Description
The Registered Nurse - Referral Coordination Team serves as a patient advocate guiding the Veteran through the healthcare system; coordinates the efforts of the care team to ensure the initiation of appropriate triage of consults, treatment plans, appointments, and follows up with the Veteran to ensure that plan of care was effective; as well as serves as an integral member of the treatment team responsible for helping to produce a therapeutic environment to promote optimal health status.
